Libby Dunahoo, Manager of the Advance Care Planning department for Northeast Georgia Health System, will present an advance care planning program to educate attendees about their choices in health care advocacy (for loved ones and themselves) and how to document choices in an Advance Directive. She also will discuss an initiative developed in our community to ensure the best care for patients in transition from their home to an emergency room and/or inpatient facilities.
