Advance Planning: Your Parents’ Legal and Financial Documents

Learn how to plan for your and your parents' care with key legal documents. This program covers important documents like wills, powers of attorney, living wills, HIPAA releases, and MOST forms. MOST is used in North Carolina, but other states have similar forms to guide medical decisions based on personal wishes.

Learning Objectives:

  • Choose a trusted person to speak for them if they are incapable of making their own medical decisions
  • Choose a person who understands the person’s health and well-being, respects their beliefs and values, and pays attention to key facts and details while being available to speak for them as needed
  • Communicate their wishes to their family and health care providers regarding special instructions, as well as releasing the health care agent from responsibilities, if and as needed
  • Record their wishes and place copies of documents with key entities; i.e., health care agent, family and friends, health care providers, and clergy
