The Medicare GUIDE Program: A New Funding Model for Loved Ones with Dementia

The GUIDE Model is a new program from Medicare to improve life for people with dementia and to support their caregivers. It offers care coordination, caregiver education, and time-off support to help people stay in their homes.

Program Presenter: Mia Yang, MD, MS

Dr. Yang serves in the Internal Medicine Section on Gerontology & Geriatric Medicine at the Wake Forest School of Medicine and is on the staff of the Sticht Center on Aging and Alzheimer’s Disease Prevention. She is also the Director of the Wake Forest House Call Program and GUIDE Program.
