Learn about legal documents typically associated with advance directives, including Last Will and Testament, Durable (Financial) Power of Attorney, Living Will, Health Care Power of Attorney, Health Insurance Portability and Accountability Act (HIPAA) Release; and Medical Order for Scope of Treatment (MOST) Form. While the MOST form is used in NC, other states have a similar document that provides specific guidelines as to the person’s wishes for medical intervention.
