“Care options for a loved one after hospital discharge”
Program Sponsor: Home after hospitalizations can be overwhelming for older adults to manage and may require additional help from family. Much may be needed including adherence to discharge instructions, implementing new medications or changes, purchasing durable medical equipment, on top of a possible increase in physical assistance needed for ADL or homemaking.  How can family do it all?  And what happens if family cannot?  What are the care options? This program will help family caregivers understand the care options for loved ones after they are discharged from the hospital as well as the options for paying for this care.  Here from a panel of experts representing these various services: Home care, senior living placement, palliative care, hospice and elder law. This program offers specific and practical tips that can be implemented by families to keep their older loved ones well cared for after discharge from a hospital.
