“Care options for a loved one after hospital discharge”

Program Sponsor: Home after hospitalizations can be overwhelming for older adults to manage and may require additional help from family. Much may be needed including adherence to discharge instructions, implementing new medications or changes, purchasing durable medical equipment, on top of a possible increase in physical assistance needed for ADL or homemaking.  How can family do it all?  And what happens if family cannot?  What are the care options?
